Please share your voice by taking the 2024 Northern Michigan Community Health Survey, conducted by MiThrive. This survey is a chance for you to tell us what is most important to you. MiThrive, a collaborative body that brings together cross-sector partners including local health departments and hospitals across the 31 counties of Northern Lower Michigan, is working to improve the health of communities in these counties by collecting data, identifying key issues, and bringing people together for change.

Details & Resources:
- Who Can Participate: Individuals who live, learn, work, or play in the 31 counties of Northern Lower Michigan.
- Time Commitment: The survey is estimated to take 10-15 minutes to complete.
- Submission Period: Now – October 6th
- Incentives: All participants can enter a drawing for a $50 gift card!
- Print at Home: Download, print, and fill out the paper survey. Once complete, return to a MiThrive partner organization.

- Please only take this survey once and if you are a resident or work primarily in one of the following Michigan counties: Alcona, Alpena, Antrim, Arenac, Benzie, Charlevoix, Cheboygan, Clare, Crawford, Emmet, Gladwin, Grand Traverse, Iosco, Isabella, Kalkaska, Lake, Leelanau, Manistee, Mason, Mecosta, Missaukee, Montmorency, Newaygo, Oceana, Ogemaw, Osceola, Oscoda, Otsego, Presque Isle, Roscommon, or Wexford.
