We Need Your Input

Learn about the 2024 MiThrive Community Health Assessment results and provide feedback on the proposed 2025 DHD#10 Community Health Improvement Plan. View the flyer below for details or click here to download PDF. 

Social Determinants of Health Inventory Scan- The Social Determinants of Health Inventory Scan will assess current efforts of service navigation and support related to Social Determinants of Health (SDoH). Our intent is to develop a better understanding of which organizations in our region are screening for the SDoH and providing informal and formal agency/organizational support to navigate clients to resources in the community.

NCCHIR is asking that a coordinator, supervisor, or someone in upper management respond to these questions. We would like one survey per program/service from each agency/organization. However, we understand if one or more individuals need to fill out this survey to give us the full picture of the work. When possible, please collaborate with your team to fill out this survey. Paper copies are available to share the survey with your team, but a final digital copy needs to be submitted. This survey will close October 6th 2025.

The MiThrive Data Briefs provide regional-level data from the 2021 MiThrive Community Health Assessment organized by significant health need.

2021 North Central MiThrive Data Briefs

The Community System Assessment focuses on organizations that contribute to community wellbeing. It answers the questions, “What are the components, activities, competencies, and capacities in the regional system?” and “How are services being provided to our residents?” The MiThrive Community System Assessment 2021 Reports are organized at the regional level.

The Forces of Change Assessment aims to answer the following questions: “What is occurring or might occur that affects the health of our community or the local system?” and “What specific threats or opportunities are generated by these occurrences?” The MiThrive Forces of Change Assessment 2021 Reports are organized at the regional level.
