• Who Can Participate: Individuals who live, learn, work, or play in the 31 counties of Northern Lower Michigan.
• Time Commitment: The survey is estimated to take 10-15 minutes to complete.
• Submission Period: Now – October 6th
• Incentives: All participants can enter a drawing for a $50 gift card!
• Print at Home: Download, print, and fill out the paper survey. Once complete, return to a MiThrive partner organization.
